Gynecological issues

The current issue highlights the diversity of contemporary gynecological research, bringing together review articles, clinical case reports and experimental studies that address both common conditions and challenging clinical scenarios, while emphasizing the importance of evidence-based, individualized patient care.

Uterine fibroids remain one of the most prevalent benign gynecological conditions, with a substantial impact on women’s health and quality of life. The review included in this issue provides a comprehensive overview of current evidence regarding their diagnosis and management, emphasizing the ongoing shift from conventional surgical approaches toward medical therapies and minimally invasive interventions. Particular attention is given to individualized treatment strategies based on symptom severity, reproductive plans and associated gynecological conditions, reflecting the principles of modern personalized treatment.

The spectrum of gynecological pathology is further illustrated by a case report describing an atypical presentation of a hydatidiform mole, a rare but clinically significant form of gestational trophoblastic disease that requires prompt recognition and appropriate management to prevent severe complications. The report highlights the diagnostic challenges posed by nonspecific clinical and ultrasonographic findings in a perimenopausal patient, and underscores the importance of maintaining a high index of suspicion in women presenting with abnormal uterine bleeding at the extremes of reproductive age, where timely diagnosis and individualized treatment are essential for achieving favorable outcomes.

Reproductive medicine remains another area of active investigation. The narrative review dedicated to recurrent pregnancy loss addresses the growing recognition of immune-mediated mechanisms among cases of unexplained miscarriage, and examines the current evidence regarding the role of intravenous immunoglobulin (IVIG) as an immunomodulatory strategy. By discussing its biological rationale, clinical efficacy and the controversies surrounding its use, the review provides an updated perspective on a topic of considerable clinical interest.

This issue also includes an experimental study focusing on ischemia-reperfusion injury, an established model for investigating tissue protection and oxidative stress in reproductive organs. The authors evaluate the potential antioxidant effects of erythropoietin on the female internal genitalia in a rat model, integrating histopathological findings with inflammatory and oxidative stress biomarkers, thereby contributing valuable preclinical data that may support future translational research.

Finally, surgical innovation is represented by an article that examines the role of a repetitive, controlled suburethral plicature procedure in the management of stress urinary incontinence. Recognizing that this condition continues to pose significant therapeutic challenges, particularly in patients with associated pelvic floor defects, the authors emphasize the anatomical and reconstructive principles underlying pelvic floor repair.
